Cleaning the Condenser Coils

To begin, shut of the power to the central air unit and turn the thermostat up. Remove the exterior metal grille if necessary, which is usually held on with bolts or screws. Use plastic bags to cover the compressor, motor, and other electrical parts, and then seal them with tape. Gently brush off any visible dirt from the condenser coils, and then use a garden hose to wash the coils from the inside. Allow the unit to dry before taking the plastic bags off of the compressor, motor, and other electrical parts.

Cleaning the Evaporator Drain

The evaporator coils on most central air conditioning units are sealed and can only be accessed by a licensed technician. But you can keep the evaporator drain clean. This is the drain that carries away moisture from the evaporator coils. If you see that there are puddles of water beneath the coils, then the drain pipe is likely clogged with bacteria and algae. Simply disconnect this drain pipe and flush the trap with a hose. Then pour in a tablespoon of bleach and re-attach.

The First 100 Days Of My Iron Palm Training

I have been training iron palm for over 100 days now. As you probably know, iron palm is a kung fu training program designed to toughen the hand so you can hit harder without injuring yourself.

There seems to be some special significance about 100 days when it comes to this. I have even seen programs called things like “iron palm in 100 days” that are supposed to teach you a quick method that will have you breaking bricks like a pro in only a few months.

My experiences have been a bit different, but I’m also taking my time and going slower rather than faster. I know that you can injure yourself if you try to progress too quickly (I’ve even done it before) and so I started very slow and soft and am slowly increasing over time.

I’m beginning with an iron palm bag (a canvas bag) full of mung beans

I’ve been keeping a log of my training. Here is a summary of the first 100 days or so.

The first 10 days were me just getting started and getting used to the program. I started doing each strike only 20 times with each hand. I let my hand fall onto the iron palm bag rather than hitting it by using muscle force. I know there’s a traditional reason to do it this way, but I’m just doing it to take it easy.

By the 30th day it had become a part of my routine. I no longer felt like it was something that was new, but instead was just another part of my training. My hands started to feel different, too. For lack of better description, they felt like they had more “energy” than before. It’s entirely possible this was just in my head, since I had been spending more time thinking about my hands, too.

By day 40 I had increased the repetitions to 25 reps of each strike with each hand. Training was fun and I looked forward to it.

While doing the qigong portion of the training on day 47 I experienced some strange sensations in my arms that felt like rushing waves. I’m not sure what to make of it, and while I know a lot of people would say it’s the qi flowing through my arms, I’m not so sure I believe that.

That same sensation happened again on the 51st day.

By the 8th day I had increased the reps to 35 of each strike per hand. I’m feeling like I’m making progress at this point, and I don’t feel it as much in my hands when I hit the bag.

Day 100! I feel like I am through the beginner phases and on my way to good conditioning. Eventually I will move on to a bag full of gravel which is like an intermediate bag.

I didn’t see 100 days as the goal. I saw it as the beginning. I’m now ready to take my training to the next level.

Installing Vinyl Replacement Sliding Glass Doors

Last week I told you how to remove your old sliding glass door in preparation for the installation of the vinyl replacement door. Let’s go through the installation procedure for both the Retrofit style and replacement style frames.

Let’s start with the retrofit frame. Like the retrofit window, the door will have a 2″ lip flush with the outside edge of the frame. The difference between the retrofit window frame and the retrofit door frame is the absence of the lip on the bottom of the door due to the sill track. Your door will have the lip on both sides and across the top. The majority of the installation process is the same whether it’s a retrofit frame or replacement frame. The first thing you want to do is check the floor for a level condition. If the base is not level, add shims to create a level base. This step is very important, because if you have a sloping bottom, the whole frame will be thrown out of square. You will have problems locking the door as well as air infiltration trouble. So, put down shims to make the base level. If possible, use one long shim to create a solid base for the weight of the door. Before setting the door in place, put down a bead of liquid nails from end to end, set the shims on top of the liquid nails, and run a generous bead of caulk on the outside face of the opening, where the retrofit lip is going to contact the exterior surface. Caulk the sides and the top.

Now set the door in place, being careful to lift the door and set the bottom track on top of the shims, rather than dragging the bottom. Obviously, this requires two people, one on each side. You tilt the top away from the opening as you set the bottom in place. Then, raise the top until the retrofit lip contacts the exterior surface where you applied the caulk. The reason I use liquid nails on the bottom rather than caulk is because I don’t like to put screws in the bottom, and the liquid nails product is as good as screws when it dries. Have your helper hold the frame in place while you go inside to plumb and secure the door. Put a level on the jamb where the slider will lock. Plumb the jamb and install shims where you put your screws. You should put one about 6 inches from the top and one 6 inches from the bottom. A good quality door will have long screws shipped with the door that are replacements for the small screws holding the locking hardware to the jamb. Be sure to shim where these screws install. Now, if you leveled the bottom and plumbed the side, your top and opposite side should automatically be square. Put shims on the opposite jamb, and drive in your installation screws. Put 3 to 4 screws in the jamb. Then, run your tape measure from the top right corner to the bottom left corner. Write down that measurement. Measure from the opposite corners, and the measurement should be the same if your frame is square. Finally, put one screw in the top center You really don’t need any more than one. You don’t need to use shims since there won’t be any load or torque on the top header. Just don’t overtighten the screw, otherwise the frame will bow.

At this point you should have the frame installed in the opening with the fixed panel already glazed in from the factory. Here is where you want to install the sliding panel. It goes in from inside the house. Set it on the side and spray a generous amount of lubricant to the steel rollers to help them spin freely. Wipe away excess before putting the panel in. Remember to raise the top into the header as far as it will go, then swing the bottom over the lip and set it down on the track. If the bottom won’t get over the bottom lip, you will have to adjust the bottom rollers to bring them up into the frame. You should see a hole on the side rail to insert a screwdriver and turn the adjustment screw counterclockwise to bring the rollers up and out of the way. After you set the rollers on the track, be sure to adjust them back down before sliding the door. Have your helper lift the side up while you turn the adjustment screw clockwise. Do both sides, then slide the door closed and check for equal daylight from top to bottom. If there is more daylight showing on top or bottom, adjust the rollers to bring the door panel plumb with the jamb. Adjust the lock. Each door should come with lock adjustment instructions. Install the screen door on the outside. Now seal the crack between the retrofit lip and the exterior surface. Go inside and fill the space between the frame and wall studs with R-13 insulation. The last step is trimming the inside. We sell vinyl trim pieces that are designed to trim the inside without having to paint, stain, or nail. Now, the replacement frame installation process is the same, except there is no retrofit lip. Therefore, when you put the frame into the opening, you have to plumb the side jambs from two places rather than one. In addition to plumbing the jamb left to right in the opening, you also have to plumb inside to outside because you don’t have the retrofit lip to keep your frame aligned with the outside wall. Also, you need to apply trim to the exterior as well as the interior. You can use a basic vinyl flat trim for the outside, and that is available on the shopping page as well. Be sure to caulk the outside trim where it meets the surrounding surface.You just installed a vinyl sliding glass door. I will be on vacation next week, but the following week’s article will deal with the best and safest way to install replacement windows on the second floor.

99495-99496: Two New Codes to Report Transitional Care Management (TCM) Services

Primary care specialties will receive the largest increase in payment by virtue of a new payment for managing a Medicare beneficiary’s care when the beneficiary is discharged from an outpatient hospital observation, inpatient hospital, community mental health center, partial hospitalization services or from an SNF. While announcing its new policy, CMS acknowledged that the extensive non-face-to-face care coordination provided by physicians and nurses was not considered in the existing payment schedule for E/M (Evaluation & Management) services. The new directive will provide payments for physicians as well as other healthcare providers for coordinating care transitions of Medicare beneficiaries after they are discharged from hospitals/skilled nursing facilities to assisted living facilities or their own homes. The new rule is effective from January 1, 2013.

The New Codes: 99495 & 99496

CMS has a clear objective in introducing these new codes for Transitional Care Management (TCM) services. They are intended to prevent emergency department visits and re-hospitalizations during the first 30 days after discharge. Apart from primary care physicians who would be billing for most of these services, specialists who provide necessary services can also bill these new CPT codes.

TCM Code Requirements

  • 99495, TCM: Communication (direct contact, telephone, electronic) with the patient and/or caregiver within two business days of discharge; Medical decision-making of at least moderate complexity during the service period; face-to-face visit within 14 calendar days of discharge.
  • 99496, TCM: Communication (direct contact, telephone, electronic) with patient and/or caregiver within two business days of discharge; Medical decision-making of high complexity during the service period, face-to-face visit within seven calendar days of discharge.

It is to be noted that both these codes necessitate communication with the patient and/or care provider within two business days of discharge, plus a face-to-face visit with the patient within a fixed time period. Decision regarding medication and management must be made at least by the day of the face-to-face visit.

Non face-to-face care coordination services can be carried out by the provider and/or licensed clinical staff under his/her direction. However, the face-to-face visit is to be performed by the providers themselves with staff assistance.

Fee Schedule for the New TCM Codes

The values assigned to the new TCM codes are 4.82 relative value units for Code 99495 and 6.79 relative value units for Code 99496. Provided the Congress prevents the impending 26.5% cut to payments for physicians and maintains the current conversion factor of $34.0066, the payments for these codes will be:

In non-facility (Physician office) settings:

  • Code 99495: $163.91
  • Code 99496: $230.90

In facility (Outpatient hospital) settings:

  • Code 99495: $134.67
  • Code 99496: $197.58

These codes can be billed only after at least 30 days post discharge, when the service period is completed. The primary care incentive payments will not be added to these amounts.

Points to Keep in Mind

  • Make sure that you bill only for post-discharge patients who require moderate or high-complexity medical decision making.
  • The initial face-to-face visit need not necessarily be in the office.
  • The first face-to-face visit with the patient after discharge is part of the TCM service and cannot be reported separately. E/M services provided additionally can be reported separately.
  • Documentation guidelines for E/M are not applicable to these codes. Providers must therefore take into account how they would like to document the non face-to-face services that are required by codes. Complexity of the medical decision making, timing of the first communication after discharge, and date of the face-to-face visit will have to be documented.
  • Providers can use these codes to bill for new as well as established patients.
  • Discharge services and the face-to-face visit required under the TCM code cannot be provided on the same day. However, the same practitioner who bills for discharge services can also bill for TCM services. Importantly, the same practitioner cannot report TCM services provided during a post-surgery period for a service with a global period since it is understood that these services are already included in the payment for the underlying procedure.
  • A very important point to remember is that only one practitioner can bill for TCM services during the 30 days post discharge of a patient. The first practitioner to bill for the service alone will receive reimbursement. Therefore, practitioners should necessarily communicate with the patient and/or caregiver, and the discharging physician to be clear about who will be managing the TCM services.
  • Practitioners can bill for TCM only once in the 30 days after discharge even if the patient happens to be discharged 2 or more times within the 30-day period.
  • Providers cannot bill for other care coordination services (such as care plan oversight codes 99339, 99340, 99374 – 99380) provided during the TCM period.

2. Before booking your trip make sure that your visa for the UK is valid for at least 3 months after you are coming back from your journey from a Schengen country.

3. For most Schengen visas you can submit an application within 3 months before your trip date. It is advisable to submit your application at least two weeks before your journey starts, but for some nationalities it may take between 3 and 4 weeks to process the visa application.

4. Your passport must be valid for at least 3 months after visa expiry date, and it must contain two blank pages for visa stamps. To get a multiple entry visa your passport must be valid for at least a year or longer from the date your journey ends.

5. If your passport was issued 10 years ago, you would need to obtain a new passport to apply for a Schengen visa. If your passport has been issued within a year, provide your old passport with the current one.

6. For most Schengen visas you can apply only after you have been in the UK for at least one month, but if you need to get a Schengen visa for Spain – you will have to wait for 6 months before you can make your first application.

7. Book appointment with the relative consulate or visa application centre well in advance. During summer time or before the bank holidays it may come to a month waiting time for an available appointment.

8. Correct documentation is key to successful visa application. You must bring your passport (s) with valid visa for the UK stamped inside the passport, photos, visa application form, employment letter (or another form of employment confirmation), pay slips, bank statements, hotel confirmation, flight confirmation, and travel insurance. If you are on a dependent visa, make sure you take your spouse passport and marriage certificate with you.

Backpacking: Tent Features

When considering buying a backpacking tent today it can be overwhelming with all the choices that are available. To complicate matters there are features that may give you a headache trying to figure out which is better. So today my aim is to give you some clarification on features to aid you in your choice. Let’s begin by looking at whether you need a three or four season tent.

The three season tent is good for summer, spring, fall, and is the normal choice for these seasons. However, if backpacking year-round and in the snow is your desire then a four season tent will be necessary. Next decide whether you want a freestanding or non-freestanding tent. The difference in these two is that the free-standing tent can stand on its own and does not need to be staked down. The free-standing tent is also easier to set up and move around if necessary. However, if windy conditions exist it is recommended that you stake your tent down so it does not blow away. Another advantage of the freestanding-tent is when it comes time to break down camp. You can point your tent opening toward the ground, shake and all the loose debris will fall right out of your tent.

Next you would determine whether you would like a single or double wall tent. In looking at a double wall tent you will find that it consists of a body and fly to cover it. Advantages of the double wall is they are usually less expensive, better ventilation, and drier in the wet conditions. In comparison the single wall tent does not have the luxury of an inner body. The benefits are easier set up and lighter to carry. However, the down fall to the single wall tent is poor ventilation which can cause a buildup of condensation which can become a problem. Finally, we turn our attention to the design of these walls and floor of the tent.

On the two wall tents a portion of the inner body is made out of mesh. This is the reason that the circulation is so much better. You can find a partial or substantial amount of mesh on your tent body. If you enjoy gazing at the stars then all you have to do is pull back your fly and experience it from the comforts of your tent. If it is rainy then pull the fly back over for full protection from the weather. It is worth mentioning that you can have a no-see-um mesh for your tent. Very small two-winged flies found all over the world are known as biting midges, this includes no-see-um, sand flies, and other tiny bugs.
